Sage Active Clinic

Sage Active Clinic

215 Bay Street, Brighton VIC

Sage Active Clinic in Brighton provides Exercise Physiology services delivered by Accredited Exercise Physiologists for adults who need clinical, individually prescribed exercise to manage their health, improve physical function and achieve measurable outcomes. We support people with chronic and complex conditions, those undergoing significant weight loss, people recovering from illness, surgery or injury, and those wanting to maintain strength, mobility and independence. Our supervised clinical exercise environment provides greater clinical oversight and individualisation than a conventional gym setting. Our clinical services include: • Chronic and complex conditions: Type 2 diabetes, cardiovascular and respiratory conditions, neurological conditions, osteoarthritis, osteoporosis, persistent pain, cancer-related fatigue, deconditioning and recovery following surgery or injury. • Older adults and healthy ageing: Supporting strength, balance, mobility, falls prevention and maintaining independence. • Patients using GLP-1 and related medications: Individually prescribed resistance and aerobic exercise to help maintain muscle mass, strength and physical function during significant weight loss, working alongside the patient’s GP or specialist. • Objective InBody 770 assessment: An InBody 770 body composition assessment is included as part of the initial assessment, providing data beyond weight alone, including skeletal muscle mass, lean body mass, body fat, visceral fat, segmental lean analysis and mineral measures, including estimated bone mineral content. Repeat assessments can be used to track meaningful changes over time, giving GPs and specialists objective information to support ongoing clinical management. • Measurable clinical progress: Strength, functional capacity and body composition can be reassessed over time. Where appropriate, we provide the referring GP or specialist with relevant assessment findings, progress and outcomes to support ongoing clinical management. For patients referred under a GP Chronic Condition Management Plan, we report following the initial assessment and again at the completion of the referred treatment period. • Individual and supervised care: Patients begin with a one-to-one clinical assessment and individually prescribed program, with one-to-one or closely supervised small-group exercise available as clinically appropriate. • Flexible access and funding: In-clinic appointments, home visits and telehealth are available. We support GP Chronic Condition Management Plans, Medicare-supported Type 2 Diabetes group Exercise Physiology services, My Aged Care, NDIS, DVA, TAC, WorkCover and private health insurance. Eligible GP Chronic Condition Management Plan appointments are bulk billed. Our approach is simple: assess, prescribe, measure and communicate meaningful outcomes back to the treating team.
